The Political Reform Act of 1974 requires that officials and employees who are designated in the agency's conflict of interest code must file a Statement of Economic Interests Form 700 each year. Form 700 publicly discloses personal assets and income. The period covered by this annual statement is January 1, 2009 through December 31, 2009.
All SWACC Board Representatives and Alternates are required to file a Form 700. The Manager, Keenan & Associates per the “Conflict of Interest Code Policy” is the designated filing agency so upon completion of the form they are to be mailed to Colleen Bjerknes, JPA Account Manager for housing of the original signed forms.
Instructions for completion:
The following instructions are intended to give general guidance and do not supersede any instructions contained within the Form 700 itself.
Using the interactive form type and complete the form online or print out and complete in black ink.
The “Cover Page” has been started for you:
- Office, Agency or Court – this has been filled in for you with the JPA name.
- Jurisdiction of Office – this has been filled in as “State”
- Type of Statement – you need to complete.
- If you became a Board member after January 1, 2009 then you need to check thefirst box “Assuming Office” and fill in the date you became a Board member.
- If you were a Board member the entire year check the second box “Annual”.
- If you are no longer a Board member, but were at some time during the 2009 year, check the third box and fill in the date you left.
- Schedule Summary – you need to complete by checking any schedules that apply to you, if none check the box “No Reportable Interests on any schedule” and enter a “one” by the total number of pages. If any of the schedules do apply to you check that box and turn to the appropriate page to complete that Schedule. Once completed count the number of pages and enter in on the Cover Page.
- Verification – Original Signature required.
